At UConn Health, our mission is to provide compassionate, high quality care in an efficient manner. Our clinical staff consists of licensed and advance-credentialed physical, occupational, and speech therapists. We treat a wide variety of patients and diagnosis, from sports injuries to complex medical conditions.
New Patient Forms
All new patients must complete a patient intake form, and one form specific to your pain area. Please download the necessary forms for your initial visit. Then print, complete, and bring them with you to your first visit.
Choose the form that applies to you based upon your area of concern/pain:
- Back Pain
- Hip, Knee and Ankle Pain
- Incontinence
- Neck Pain
- Shoulder Pain
- Vertigo/Dizziess
- Wrist and Hand
As part of the team at UConn John Dempsey Hospital, our staff works side-by-side with the physicians to not only care for patients, but to also improve patient care through research initiatives.
Scheduling
We understand that your time is valuable. Your evaluation and treatment appointments are times reserved especially for you and are part of a full schedule for our therapists. We request that you provide at least 24 hours notice for any cancellations.
Please complete all forms prior to your appointment. This will allow us to utilize your treatment time efficiently.
