The physical therapy and rehabilitation team at UConn Health provides a wide range of services to help you regain mobility, manage chronic pain, or recover from injury. The rehabilitation therapy is offered in a friendly, relaxed environment where patients can access private treatment rooms and three treatment gyms, as well as dedicated free parking.

New Patients
All new patients must complete a patient intake form, and one form specific to your pain area. Please download the necessary forms for your initial visit. Then print, complete, and bring them with you to your first visit.
