    The nurse did not look at my chart at all prior to the appointment, it would be beneficial if they spent ar least 5 seconds to quickly glance through a chart prior to taking vitals and history

    I unfortunately can't remember her name, but the medical assistant who worked with Dr. Ulrich was fantastic! She was very kind and helped me a lot through an uncomfortable visit. I appreciated having her support.
