Your Visit Notes Available in MyChart

Users of MyChart, UConn Health’s secure, online patient portal, have access to the notes that a health care provider writes during their appointment. These visit notes contain useful information such as a summary of the visit, what was discussed, diagnosis, and treatment plan.

  • What Are the Benefits of Reading My Visit Notes?

    Studies have shown that when patients have access to their provider’s notes, they:

    • Have an easier time recalling what was said during the appointment
    • Are more engaged in their care
    • Have a better understanding of their medical conditions
    • Take medications as prescribed
    • Feel better informed when making decisions about their health
  • How Can I Access My Visit Notes?

    The easiest and most convenient way to access your medical records, including the visit notes, is to log into your MyChart account. MyChart is UConn Health’s free, secure online patient portal that lets you access your medical records, request an appointment, send and receive messages to and from your provider, and access your test results from your computer or mobile device. Signing up for MyChart is simple and only takes a few minutes. To enroll, visit MyChart Patient Portal.

  • When Can I See My Test Results in MyChart?

    Generally, your test results are released to your MyChart account as soon as they are finalized.

  • Will I Be Able To See Older Notes & Test Results on MyChart?

    Most results and notes will be released beginning at noon on November 2, 2020 and going forward.

  • Why Are Some Test Results Not Shared Electronically via MyChart?

    If your provider has any unique patient safety concerns related to your results, they will be contact you directly. In a small number of cases, the results might be considered sensitive and will not be released to the portal immediately. Your provider will contact you to discuss those results.

  • If I Prefer To Receive Test Results Directly From My Provider & Do Not Want To See Them Before My Next Visit, Can the Results Be Blocked From MyChart?

    Yes. If you prefer not to see a particular test result or visit note on MyChart, ask your provider to block the result or note. The provider can release the test result or visit note to MyChart at a later time upon your request.

  • I Have a MyChart Proxy. Can They See My Visit Notes?

    If you have designated a MyChart proxy (someone you have authorized to view your health information on MyChart on your behalf), your proxy will also have access to this additional health information.

  • Can I Change or Terminate My MyChart Proxy?

    If you have concerns about your MyChart proxy seeing sensitive or other health information about you, you may terminate your MyChart proxy’s access or select another individual to serve as your MyChart proxy.

    If you would like to change or terminate your MyChart proxy, you may do so within MyChart or by contacting UConn Health’s Health Information Management Department at 860-679-3380.

  • What if I Don’t Understand Something in the Visit Notes?

    If you have any questions about the information in your medical record, please contact the relevant provider.

  • Can I Still Get a Copy of My Full Medical Record?

    For copies of your medical record, please contact the Health Information Management (HIM) Department (Monday-Friday 7:30 AM to 5:30 PM) at 860-679-2787, or visit our Patient Resources page.
